Program FYI Form

This form should be completed when your program doesn't need approval from Admin. That is the case when:

• The program does not need a budget (supplies, rentals, honorarium etc)
• The program is only focused on participants from your branch (not online, not a "big draw" that could bring in folks from other branches like an author talk)
