Huron County Library Program Assessment surveys

These four surveys are the primary way the library gains feedback from program participants. There is one survey for each of our 4 Strategic Programming Categories. The titles of the questionnaires are identical except for a number because naming the questionnaire "reducing isolation" or "youth become lifelong library users" may be off-putting for participants, or even skew responses.

1. Reducing Isolation
